Do I have to request this PS3 disc, or is this automatically sent to all subscribers?

You have to request it. There was a link straight to it in the original press release. Probably something you can search for on Netflix too. Playstation 3 Instant Streaming Disc or something. Eventually tho, it will be built in. Netflix is working with Sony to include it in a firmware update.

One of the movies I tried to watch had some major audio/video sync issues. Audio was delayed by about five seconds. I've heard of some other people having this issue, but it's not with all movies.

Default Playon

Don't know if this was mentioned yet, but if you have a PC, there's a way to stream Netflix, Amazon Video on Demand, Hulu, CBS, ESPN, CNN, Revision3, and more to your PS3, Xbox360 AND wii right now. It's called Playon. I'm getting my PS3 disk in the mail this week as well, but I've been watching Netflix and more from my PS3 for weeks now. Here's the URL: http://www.themediamall.com/playon/

Since this streams the video to your PS3 through the PC, how does this handle the "No HD to PC" rules that some content providers like Netflix run with?

For example, I can receive "Stargate: The Ark of Truth" on my PS3 in HD from Netflix, but only the SD version is available when I try to access it from my HTPC.

Yep.. same. Netflix on the PS3 is superior.
