The link is legit. Disney is now taking replacement requests through that form, and also by phone:

If you have any other questions please you may also contact us at 1-800-723-4763, Monday's 9:30-7:30 and Tuesday through Friday 9:30-6:30 CST

I called them over a week ago. I just checked with them again and was told I'd have my replacement in 7-10 business days. Cool!

It's good to see that Disney has started the replacement program.

Now the question is, have they pulled the mis-framed version from the shelves and replaced them with the proper version. I was waiting to get the trilogy until after the replacement was in place, and think it would be easier to now just get the proper version, instead of doing a mail-in replacement.

yea i called and was on the phone for a couple of minutes, they took down my info and said i should have it in about 2 weeks

1-800-72-DISNEY is the number, however it's best to pursue it electronically due to heavy call volumes.

I had no idea that Disney would replace a damaged DVD! I love Disney.

"If you accidentally damage or break one of your Disney DVDs, you can get a replacement disc for a nominal charge of $6.95. ...."

Do all studios do this?

I'm also very impressed with how they are handling the POTC1 Blu-ray issue as well.
