Default $10 rebate on Con Air, The Rock or Crimson Tide

Get $10 back for upgrading from your normal DVD to the Blu Ray version.

credit goes to Mr.NightLife over at SD for this info:

"i just googled it and i think it comes with the blu-ray (in the packaging)

A quick note to beware of a couple items on the $10 "same title" rebate for purchasing the BD of Con Air, The Rock or Crimson Tide (send in the UPC from the DVD and BD and get $10 back):

1. The rebate is only good for Blu-Ray purchases between 1/8/2008 and 12/31/2008.

2. The purchase of the Blu-Ray must have a store receipt "from the same state as the consumer address." This means no rebate from most internet retailers unless you happen to live in the same state as say, Amazon."

The offer form states that you can use the proof of purchase from "any" edition of the sd counterpart. However, on my Criterion version of The Rock there isn't a proof of purchase. I wonder if they'd accept the bar code?

I bought the Rock BD from walmart yesterday and there was no $10 rebate coupon in the box. I did get a card that indicated I should use Monster HDMI cables!

For those who got the rebate form in their blu-ray case:

Is there a number or anything on the form for customer service? I did not receive the rebate form and am hoping to acquire one.

Any help is greatly appreciated.
